Create contact forms, surveys, registration forms, and multi-step forms in minutes. Off until you switch it on. #### WooCommerce integration Turn it on in **MIS Webform Settings WooCommerce**. It needs WooCommerce 8.2 or newer to be installed and active; without WooCommerce, nothing changes and the settings card shows how to install or activate it. * **Product enquiry forms:** show a form in a product tab or under the Add to cart button. * **Product-specific forms:** pick a different form, or none, for any product. * **Automatic product details:** the product, chosen variation, SKU and price are saved with the enquiry. No hidden fields to set up. * **Product customization fields:** add fields above Add to cart (engraving text, gift message, size notes and more). Required answers are checked, and the answers travel with the item into the cart, checkout, order emails, My Account and the order screen, and (optionally) under MIS Webform Submissions. Prices don’t change. * **Cart and order details:** save the visitor’s cart, or the order when the form is shown on the order confirmation page or in My Account. The submission is linked from the order’s edit screen. * Works with the Cart and Checkout blocks and High-Performance Order Storage (HPOS). Switching the integration off, or deactivating WooCommerce, pauses these features. Your forms, product choices and saved submissions are kept. #### Field types Text, email, phone, number, website, date, time, dropdown, multi-select, radio buttons, checkboxes, toggle, name, address, country, star rating, slider, Likert scale, color picker, signature, terms and conditions, hidden field, file upload, page break, and more. ### External services This plugin can connect to two Google services. Both are off by default and only work after you set them up. **Google reCAPTCHA** (optional spam protection for a form) * Sends: the visitor’s reCAPTCHA answer and IP address to Google, when they submit a form that has reCAPTCHA turned on. The reCAPTCHA script is loaded from `https:// www.google.com/recaptcha/api.js` and the answer is checked at `https://www.google. com/recaptcha/api/siteverify`. * When: only on forms where you turned reCAPTCHA on and added your own keys. * [Privacy Policy](https://policies.google.com/privacy) and [Terms of Service](https://policies.google.com/terms) **Google Sheets** (optional, MIS Webform Google Sheets) * Sends: your service account sign-in to `https://oauth2.googleapis.com/token`, and the form answers you chose to `https://sheets.googleapis.com/v4/spreadsheets/`. These answers can include personal data the visitor entered. * When: only after you connect a Google account and turn on a feed for a form. * [Privacy Policy](https://policies.google.com/privacy), [Terms of Service](https://policies.google.com/terms), and [Google APIs Terms](https://developers.google.com/terms) **WooCommerce** (optional) is another plugin on your own site, not an external service. When you turn on **Save cart contents** or **Save order details** for a form, the cart items or the order number, status, total and items are saved with that form’s submissions on your site. Nothing else is sent anywhere. Go to **Plugins Add New**, search for **MIS Webform**, then install and activate it. 2. Go to **MIS Webform New Form** and build your form, or pick a template. 3. Add the form to a page with the **MIS Webform** block, or paste its shortcode, for example [mis-webform id=”1″]. ## FAQ ### How do I add a form to a page? In the block editor, click **+**, search for **MIS Webform**, and choose your form. In other editors, paste the form’s shortcode, for example `[mis-webform id="1"]`. ### Where can I see the submissions? Go to **MIS Webform Submissions**. You can read them, download uploaded files, delete them, and export them to CSV. ### How do I send submissions to Google Sheets? 1. Go to **MIS Webform Google Sheets** and add your Google service account key. The page shows step-by-step how to get one. 2. Share your spreadsheet with the service account email as an **Editor**. 3. Create a feed: choose a form and a spreadsheet, then save. Each new submission is then added as a new row. The free version connects each form to one spreadsheet. ### What is the signature field? It lets visitors sign your form with their mouse or finger, like signing on paper. The signature is saved as an image with the submission. ### What does the dashboard show? How many people viewed each form, how many submitted it, and the conversion rate, for the last 7 days. No personal visitor data is stored for this. ### Are uploaded files safe? Yes. Uploaded files are stored in a protected folder with random names, and only administrators can download them. On Nginx servers, add this rule: location ^~ / wp-content/uploads/mis-webform-private/ { deny all; } ### My site uses Cloudflare. Why do all submissions show the same IP? Go to **MIS Webform Settings Visitor IP address** and choose **Cloudflare**. ### How do I add an enquiry form to my products? 1. Turn on the integration in **MIS Webform Settings WooCommerce**. 2. Create a form (the **Product Enquiry** template is a good start) and choose it as the **Enquiry form on product pages**. For fields shoppers fill in before buying, choose a form (the **Product Customization** template is a good start) as **Customization fields on product pages**. 3. To use a different form on one product, edit the product and choose it in the ** MIS Webform** box. Block theme? Add the shortcode `[mis-webform-product-enquiry]` to the product template. ### What happens if I deactivate WooCommerce? The WooCommerce features stop, and nothing else changes. Your forms keep working, and store details already saved with submissions are still shown on the Submissions screen and in CSV exports. ### Will I lose my forms if I delete the plugin?
